Requirements

STEP 1: All registrees should read and agreed with our rules and regulation. For Players, download and sign the waiver form and submit it in our committee.

Team Application

STEP 2: Finalize your lineup first. Register your team and you will get your unique Team Code. We will notify you in via email you provided if your application is verified by the committee. Your players can use the Team Code to register under your team and division.

Player Registration

STEP 3: Ask your team representative for your Team Code to have an access in registration form. Provide your picture and vaccine card. Please secure Certificate of Residency from Office of Barangay West Crame if your registration is verified by the committee.
